Submit your request for a date and time for a phone consultation. This call will include an in-depth case history that will last approximately 30 minutes. After your phone consultation we will schedule you for a 30 minutes in-person exam including: non-invasive and state-of-the-art neurological scans, and a physical tonal evaluation.
